*The genuine fourth edition was issued in 1901, by Appleton in New York; Osler preferred to produce a new edition every three years but through an oversight, the British copyright was not obtained for the fourth edition and a pirated and cheaper version was issued by Pentland, Edinburgh, heavily affecting legitimate sales. Osler therefore rushed out a fifth edition in 1902 solely to secure British copyright.
